Embark on a dynamic per diem opportunity as a Nurse Practitioner delivering home health risk assessments across Cooper County, MO. Youll play a vital role in supporting Medicare Advantage plan members, helping to ensure comprehensive, quality care in the community. This position offers the flexibility to build your own schedule with shifts available any day of the week and accommodates a variety of home environments, including Boonville, Wooldridge, Blackwater, and Otterville.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments within patients residences
• Review and evaluate prior and current medical histories, existing medical conditions, and medication use
• Provide essential education to patients regarding their diagnoses and prescribed medications
• Identify care gaps and recommend appropriate next steps for resolution
• Focus exclusively on assessmentno requirement for treatment plans or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Missouri
• Board certification via AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited nursing program
• At least 6 months of recent nurse practitioner experience preferred
