Advance your career as a Nurse Practitioner specializing in home health with this per diem opportunity. You will perform in-depth clinical risk assessments for Medicare Advantage members in a home setting across multiple locations in Pitt County. Each assessment is anticipated to take approximately 45 minutes and may include annual health reviews or post-discharge check-ins. This role focuses on evaluating patient health while delivering education and recommendationsno prescribing or treatment plan creation required.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members
• Assess prior and current medical history, medical conditions, and medication use
• Educate members regarding health conditions and medications
• Identify care gaps and offer actionable recommendations for improvement
• Document clear and concise findings; no prescribing of medications or treatment plan authoring needed

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC required
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• Minimum 6 months experience as a Nurse Practitioner preferred

Per evaluation, youll earn between $80 and $90, plus a mileage stipend and compensation for administrative time, rewarding your clinical expertise and commitment to quality care.
