Enjoy a dynamic, patient-focused opportunity as a Nurse Practitioner delivering home health risk assessments for Medicare Advantage plan members in communities across Pitt County. This per diem position puts you in the heart of patient care, with flexible shifts and the ability to create your own schedule, ensuring optimal work-life balance. Each home visit typically requires about 45 minutes and will vary in naturefrom annual health reviews to post-discharge follow-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling for your convenience
- Minimum hourly commitment required weekly or bi-weekly
Responsibilities:
- Perform comprehensive annual risk assessments in patient homes for a Medicare Advantage health plan
- Collect detailed medical, surgical, and medication histories
- Educate patients on their medical conditions and medication regimens
- Identify care gaps and communicate recommendations for follow-up
- Focus solely on assessment and educationno prescribing or treatment plan creation required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- CPR certification (BLS)
- Board-certified through AANP or ANCC
- Minimum of 6 months current NP experience preferred
- Masters degree in nursing (MSN) from an accredited institution
This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
