Take on a vital role in healthcare by performing in-home risk assessments as a Nurse Practitioner for Medicare Advantage plan members. This per diem opportunity offers flexibility and the ability to create your own schedule, supporting patients across Chautauqua County. Shifts are flexible and can be scheduled between Monday and Sunday, with a minimum weekly or bi-weekly hour commitment.
Schedule:
- Flexible shiftswork any day Monday through Sunday
- The ability to set your own schedule
- Minimum commitment per week or every two weeks
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, medical conditions, and medications
- Educate patients on their health conditions and medications
- Identify gaps in care and provide appropriate recommendations
- Visits may include annual health assessments or post-discharge check-ins
- No responsibility for treatment plan development or medication prescribing
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR (BLS) certification
- Board Certified through AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited program
- At least 6 months of recent Nurse Practitioner experience preferred
This position provides competitive compensation: earn between $80 and $90 for each completed evaluation, with additional mileage reimbursement and administrative time compensation.
