Job Description

Seeking an experienced Nurse Practitioner to provide home health risk assessments on a per diem basis within Chautauqua County. This role is dedicated to evaluating Medicare Advantage plan members through in-home visits, ensuring thorough documentation and patient-centered care. Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule based on your availability
  • Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct comprehensive annual health risk assessments in members homes
  • Review and document medical history, current health status, and medications
  • Provide patient education about health conditions and prescribed medications
  • Identify care gaps, making recommendations to encourage optimal health outcomes
  • Visits may include annual assessments or post-discharge evaluations
  • No responsibilities related to treatment planning or medication prescribing

Qualifications:

  • Active Nurse Practitioner license in NY
  • Current CPR certification (BLS)
  • Board Certified by AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least 6 months of Nurse Practitioner experience preferred

This opportunity offers $80$90 per completed assessment, along with mileage reimbursement and compensation for administrative time.
