Seeking a skilled Nurse Practitioner with home health experience to conduct risk assessments in Harrison County, WV. Candidates will travel to patient homes throughout communities such as Clarksburg, Bridgeport, and Shinnston. This per diem role offers exceptional flexibility and autonomya perfect fit for practitioners who enjoy working independently and serving Medicare Advantage members.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage patients
- Gather and analyze patient medical histories, current conditions, and medication profiles
- Deliver education to patients regarding diagnoses and medications
- Identify gaps in care and suggest actionable recommendations for improved patient outcomes
- Documentation onlyno prescribing or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in West Virginia
- Board Certification via AANP or ANCC
- MSN from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months of Nurse Practitioner experience is preferred
This position offers compensation between $80 and $90 per completed evaluation, in addition to a mileage stipend and payment for administrative time.
