Enhance the well-being of Medicare Advantage members as a skilled Nurse Practitioner conducting home health risk assessments within Craig County, OK. In this dynamic per diem role, you'll leverage your clinical expertise to perform comprehensive evaluations, provide valuable health education, and contribute to improved patient outcomesall while enjoying the flexibility to define your own schedule.
Schedule:
- Flexible shifts available seven days a week

- Minimal weekly or bi-weekly hourly commitment ensures maximum autonomy
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments during 45-minute visits, meeting Medicare Advantage plan requirements
- Review and document patient medical and medication history
- Evaluate current health conditions and discuss health concerns with patients
- Deliver clear education on chronic conditions, management strategies, and medications
- Identify care gaps and recommend appropriate next steps, supporting member health
- Visits may occur in a variety of locations within Craig County, including Ketchum, Welch, White Oak, and Vinita
- No treatment plan development or prescribing requiredfocus is on thorough assessment and education
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Minimum of 6 months NP experience preferred
