Schedule:
- Flexible shifts available any day of the week (MondaySunday)
- Opportunity to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual or post-discharge home health risk assessments in patient residences across Fairfield County, including Bridgeport, Stamford, and Norwalk
- Assess prior and current medical history, conditions, and medications during 45-minute home visits
- Educate patients on their medical conditions, medication usage, and wellness strategies
- Identify gaps in care and recommend appropriate solutions
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Connecticut
- Current BLS/CPR certification
- Board Certification as an NP through AANP or ANCC
- Graduate of an accredited MSN program
- Preferred: Minimum of 6 months NP experience in a home health or related setting
