Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduled shifts to fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Perform in-home risk assessments, each lasting approximately 45 minutes, for Medicare Advantage participants
• Review and document prior and current medical history, conditions, and medications
• Educate patients on their diagnoses and prescribed therapies
• Identify gaps in care and recommend actionable interventions
• No responsibility for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Missouri
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred
