Job Description
Advance your career as a Nurse Practitioner delivering crucial home health risk assessments for Medicare Advantage plan members throughout Miami County, Ohioincluding Troy, Piqua, Tipp City, and Union. This per diem role offers extensive flexibility, allowing you to create your own schedule with shifts available any day of the week, and minimal weekly or bi-weekly hourly commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments during 45-minute visits tailored as annual assessments or post-discharge evaluations
- Review and evaluate patients prior and current medical history, conditions, and medication usage
- Educate patients on disease management, preventive care, and prescribed medications
- Identify unmet healthcare needs, care gaps, and make actionable recommendations
- No requirements for developing treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Ohio
- MSN degree from an accredited nursing program
- Board Certification through AANP or ANCC
- Active CPR certification (BLS)
- Minimum of six months experience as a Nurse Practitioner preferred
