As a Nurse Practitioner specializing in home health, you'll serve Medicare Advantage plan members by conducting comprehensive, face-to-face visits in home settings throughout Bellingham, Ferndale, Lynden, and Everson. This per diem opportunity enables you to tailor your schedule, while performing vital assessments that contribute to exceptional patient care without the responsibility of prescribing medications or generating treatment plans.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete in-home health risk assessments, averaging 45 minutes per visit
• Assess and review prior and current medical histories, chronic conditions, and medication usage
• Educate patients on disease management and medication adherence
• Identify gaps in care, providing recommendations to improve patient outcomes
• Coverage area includes Bellingham and surrounding communities within Whatcom County
• No requirement to create ongoing treatment plans or prescribe medicines

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited program
• At least 6 months of recent, relevant NP experience (preferred)

In recognition of your expertise and dedication, compensation is offered at $80$90 per completed evaluation, which also includes a mileage stipend and payment for administrative time.
