Job Description

Delivering quality care directly to patients where they live is at the heart of this meaningful opportunity for Nurse Practitioners specializing in home health. In this flexible, per diem position, you will perform home health risk assessments throughout Sharp County, ARincluding communities such as Cherokee Village, Horseshoe Bend, and Cave Cityproviding crucial evaluations and education for Medicare Advantage plan members.

Schedule/Hours:

  • Flexible shifts available any day, Monday through Sunday
  • Design your own schedule to suit your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for a Medicare Advantage health plan
  • Evaluate patient histories, medical conditions, and medication use
  • Educate patients about their conditions and prescribed medications
  • Identify care gaps and offer recommendations for follow-up or interventions
  • Assessments may include both annual evaluations and post-discharge visits
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas (AR)
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • At least 6 months of experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
