Advance your Nurse Practitioner career in the rewarding field of home health by performing comprehensive risk assessments for Medicare Advantage plan members across Jefferson County. This per diem opportunity offers flexibility and autonomy, with the ability to create your own schedule and select preferred shifts anywhere from Monday through Sunday. Assignments are spread throughout the region, allowing you a varied and engaging caseload that includes locations such as Watertown, Adams, Fort Drum, Le Ray, and surrounding areas.

Schedule:

• Flexible shifts available seven days a week
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete in-home annual health assessments and post-discharge evaluations for Medicare Advantage members
• Obtain and review medical history, health conditions, and current medication use
• Educate members on health issues and proper medication management
• Identify care gaps and make recommendations to improve patient outcomes
• No responsibility for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in New York
• Master of Science in Nursing (MSN) from an accredited program
• Board certification through AANP or ANCC
• Current BLS CPR certification
• Minimum six months of recent nurse practitioner experience preferred
