Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This position involves performing comprehensive evaluations for Medicare Advantage members residing throughout Jefferson County, NY, including areas such as Le Ray, Watertown, Adams, and Fort Drum. Each assessment typically lasts around 45 minutes and may focus on annual health evaluations or post-discharge follow-ups.

Schedule:

• Flexible shifts available seven days a week
• Create your own schedule to best fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual risk assessments for Medicare Advantage members in their homes
• Review and document members prior/current medical history, existing conditions, and medication usage
• Educate patients on their medical conditions and prescribed medications
• Identify gaps in care and make recommendations to support optimal outcomes
• No prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in NY State
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• Minimum of six months of experience as a Nurse Practitioner is preferred

This role offers highly competitive compensation: earn between $80-$90 per evaluation, plus receive a mileage stipend and additional pay for administrative time.
