As a Nurse Practitioner, you will provide critical evaluations for Medicare Advantage plan members in various locations, including Le Ray and surrounding areas such as Watertown, Adams, and Fort Drum. This position allows for autonomy and a flexible schedule, empowering you to design shifts that best fit your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule with minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform comprehensive annual home health risk assessments in patients residences
• Review and document prior and current medical histories, chronic conditions, and full medication lists
• Deliver patient and caregiver education regarding medical conditions and treatments
• Identify care gaps and provide recommendations to improve patient outcomes
• No requirement to initiate treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in New York
• Current CPR (BLS) certification
• Board certification through AANP or ANCC
• Minimum of 6 months recent NP experience preferred
• MSN from an accredited nursing program
