Deliver high-quality patient care in a dynamic, flexible home health setting as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity empowers you to set your own schedule, supporting Medicare Advantage plan members in Sumter County, including The Villages, Wildwood, and Bushnell. Enjoy the autonomy of independent patient visits and meaningful interactions, with each assessment lasting approximately 45 minutes and focusing on prevention, education, and comprehensive health evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Essential Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of recent nurse practitioner experience preferred
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical histories, chronic conditions, and medication lists
- Educate patients about their conditions and medications
- Identify gaps in care, providing actionable recommendations to improve patient outcomes
- Focus on assessment with no requirement for developing treatment plans or prescribing medications
