You'll conduct comprehensive assessments in locations such as Berryville, Green Forest, and Holiday Island during flexible shifts from Monday to Sunday, with the ability to tailor your work commitment on a weekly or bi-weekly basis.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum hourly commitment weekly or bi-weekly
Key Responsibilities:
- Conduct annual in-home health risk assessments for Medicare Advantage plan members
- Evaluate prior and current medical history, chronic conditions, and detailed medication history
- Provide clear education to patients regarding medical conditions and prescribed medications
- Identify care gaps and recommend appropriate next steps to improve patient wellness
- Focus solely on assessmentno treatment planning or prescribing required
Required Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Minimum of 6 months of recent NP experience preferred
- Graduate of an accredited MSN program
