This per diem position offers the unique opportunity to provide vital care to Medicare Advantage members across Coffee County, TN, including Tullahoma, Manchester, and Beechgrove. Youll play a key role in supporting patient health and preventative care, all while enjoying autonomy in scheduling and a rewarding compensation structure.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit your needs
- Meet minimum weekly or bi-weekly hourly requirements
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes
- Perform thorough reviews of past and current medical history, including conditions and medications
- Deliver clear education to patients regarding health conditions and prescribed therapies
- Identify care gaps and offer recommendations to enhance health outcomes
- No requirements for treatment plan creation or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current Basic Life Support (CPR/BLS) certification
- Board Certification through AANP or ANCC
- Minimum of six months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
