Job Description

Make a meaningful impact by utilizing your clinical expertise to support the health and well-being of individuals in their homes. In this per diem opportunity, you will conduct comprehensive risk assessments for Medicare Advantage plan members, helping shape better health outcomes across Coffee County, Tennesseeincluding Tullahoma, Manchester, and Beechgrove. Each visit lasts approximately 45 minutes and may include annual wellness evaluations or post-discharge check-ins.

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Create your own work schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform thorough annual home health risk assessments as part of a Medicare Advantage program
  • Review and evaluate patients current and prior medical histories, health conditions, and medication regimens
  • Educate patients on their diagnoses and prescribed medications
  • Highlight gaps in care and offer actionable recommendations to help resolve them
  • No treatment planning or prescription-writing required

Qualifications:

  • Current Nurse Practitioner license in Tennessee
  • Active BLS/CPR certification
  • Board Certification through either the AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience (preferred)

This role features highly competitive per-evaluation rates between $80 and $90, as well as mileage reimbursement and compensation for administrative tasks.
