This per diem opportunity offers independence and flexibility for Nurse Practitioners interested in making a meaningful impact on Medicare Advantage plan members in Franklin County. Roles will support patients in locations such as Winchester, Decherd, and Estill Springs, providing valuable assessments that empower better health outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Control over your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, active conditions, and medications
- Educate patients about their health conditions and prescription regimen
- Detect care gaps and provide appropriate recommendations
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Licensed Nurse Practitioner in Tennessee
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum 6 months of experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
