Job Description

Deliver high-quality patient care by providing home health risk assessments to Medicare Advantage members in Coffee County, TN. This per diem role offers exceptional flexibility, allowing you to create your own schedule and select shifts throughout the weekMonday through Sunday. Whether you prefer a minimum weekly or bi-weekly commitment, this opportunity is tailored to fit diverse availability and work-life balance needs.

Schedule:

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members in Tullahoma, Manchester, Beechgrove, and surrounding areas
  • Complete assessments during annual health check-ups or post-discharge visits (typically 45 minutes in duration)
  • Review and evaluate prior and current medical history, ongoing conditions, and medication history
  • Educate patients about their health conditions and prescribed medications
  • Identify potential gaps in care, providing clear recommendations to support patient health
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current BLS/CPR certification
  • Board Certification via AANP or ANCC
  • MSN from an accredited nursing program
  • Minimum 6 months of recent NP experience preferred
