Make a meaningful impact by delivering high-quality healthcare directly to patients in their homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers unmatched scheduling flexibility and the unique experience of serving Medicare Advantage plan members throughout Lee County, including areas like Cape Coral, Fort Myers, and Lehigh Acres.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate prior and current medical history, conditions, and medication usage
- Provide education to patients regarding medical conditions and medication regimen
- Identify potential gaps in care delivery and offer actionable recommendations to enhance patient outcomes
- No need to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner license in Florida
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This role provides dynamic independent practice while allowing you to cultivate relationships with patients in their home environments. Expect engaging one-on-one encounters averaging 45 minutes per risk assessment, with all necessary administrative support included. Per evaluation, compensation is $80-90, with additional mileage and administrative stipends.
