Job Description

Deliver personalized healthcare while enjoying maximum flexibility with this per diem Nurse Practitioner opportunity. This role focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Preble County, Ohio, including locations such as Eaton, Camden, Lewisburg, and West Alexandria. Each assessment typically lasts about 45 minutes and may involve annual wellness exams or post-discharge visits.

Schedule:

  • Flexible shifts available any day Monday through Sunday
  • Set your own schedule to fit your availability
  • Minimum commitment required on a weekly or bi-weekly basis

Responsibilities:

  • Perform comprehensive home health risk assessments for plan participants
  • Review each patients prior and current medical history, conditions, and medications
  • Offer guidance and education to patients regarding medical conditions and proper medication usage
  • Spot care gaps and make recommendations to promote better health outcomes
  • No prescribing of medications or development of treatment plans is required

Qualifications:

  • Active Nurse Practitioner license in the State of Ohio
  • Current BLS certification (CPR)
  • Board Certified by AANP or ANCC
  • Graduate of an accredited MSN program
  • A minimum of 6 months of Nurse Practitioner experience preferred
