Job Description
Make a significant impact in patient care by performing home health risk assessments as a Nurse Practitioner, specializing in Home Health. This per diem position provides flexibility and autonomy, enabling you to create your own schedule while serving Medicare Advantage plan members throughout Preble County. Assignments may include visits in areas such as Eaton, Camden, Lewisburg, and West Alexandria, with each risk assessment averaging 45 minutes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage members in their homes
- Review patients current and previous medical histories, including conditions and medications
- Offer education on health conditions and medication management
- Identify potential care gaps and provide recommendations for follow-up
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Ohio
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited institution
- Minimum 6 months of recent experience as a Nurse Practitioner preferred
Compensation for this role is provided per completed evaluation, along with a mileage stipend and reimbursement for administrative time.
