Job Description

Play a vital role in improving community health by performing home health risk assessments with a focus on Medicare Advantage members. As a Nurse Practitioner, you will utilize your expertise in home health to support early intervention and preventive care while enjoying significant flexibility in your work schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Opportunity for a minimum weekly or bi-weekly commitment

Primary Responsibilities:

  • Conduct thorough home health risk assessments in the homes of Medicare Advantage plan members across locations in Jefferson County (including Lakewood, Arvada, and Westminster).
  • Review and analyze previous and current medical history, existing health conditions, and medication use.
  • Offer patient education about health conditions and medications to foster engagement in individual health management.
  • Evaluate and document care gaps, providing actionable recommendations to improve patient outcomes.
  • Each evaluation visit typically lasts about 45 minutesno treatment planning or prescription duties required.

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Board Certification from AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Home health knowledge and the ability to work independently
