Seeking a skilled healthcare professional to perform home health risk assessments within Jefferson County. This per diem opportunity allows you to make a meaningful impact on the lives of Medicare Advantage plan members while enjoying flexible work arrangements.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to accommodate your availability
- Minimal weekly or bi-weekly hour requirement
Key Responsibilities:
- Conduct home health risk assessments in patient residences within Jefferson County, including Lakewood, Arvada, and Westminster
- Complete comprehensive evaluations covering medical history, current conditions, and medication details
- Educate members regarding their health conditions and prescribed medications, empowering informed decision-making
- Identify gaps in care and recommend appropriate follow-up, while avoiding treatment planning or medication prescribing
- Support care transitions by performing annual assessments or post-discharge visits as needed
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
This position offers compensation of $80-$90 for each completed evaluation, plus a mileage stipend and remuneration for administrative time. This role is ideal for experienced nurse practitioners seeking flexibility and autonomy, while providing high-quality care in patients homes.
