Make a meaningful impact in patient care by conducting home health risk assessments for Medicare Advantage plan members throughout Halifax County, NC. This per diem opportunity offers exceptional flexibility, letting you customize your schedule and select shifts that suit your lifestyle including options between Monday and Sunday and a minimum weekly or bi-weekly commitment.

Key Responsibilities:
- Perform home health risk assessments, averaging 45 minutes each, at patient residences throughout Halifax County, including locations such as Littleton, Scotland Neck, Weldon, and Roanoke Rapids
- Conduct thorough evaluations of patients prior and current medical history, medical conditions, and medication regimens
- Educate members about their conditions and medications during each visit
- Identify care gaps and offer actionable recommendations to optimize patient health
- Focus strictly on assessmenttreatment plan development and prescribing are not required
Qualifications:
- Licensed Nurse Practitioner (NP) in North Carolina
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
