Experience meaningful patient interaction by providing home health risk assessments as a Nurse Practitioner in Broome County. This per diem opportunity offers flexibility in your schedule and a rewarding way to make a difference in patients lives within the comfort of their homes.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduling: create your own work calendar
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, conditions, and medications
- Deliver education to patients regarding their medical conditions and prescribed medications
- Identify any gaps in care, providing recommendations to promote better health outcomes
- Visits average 45 minutes and may involve annual assessments or post-discharge check-ins
- No need to develop treatment plans or prescribe medication
Preferred Qualifications:
- Active Nurse Practitioner license in New York State
- MSN degree from an accredited nursing program
- Board Certification through AANP or ANCC
- Current BLS certification
- At least 6 months of Nurse Practitioner experience preferred
