Job Description

Experience meaningful patient interaction by providing home health risk assessments as a Nurse Practitioner in Broome County. This per diem opportunity offers flexibility in your schedule and a rewarding way to make a difference in patients lives within the comfort of their homes.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Self-scheduling: create your own work calendar
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, conditions, and medications
  • Deliver education to patients regarding their medical conditions and prescribed medications
  • Identify any gaps in care, providing recommendations to promote better health outcomes
  • Visits average 45 minutes and may involve annual assessments or post-discharge check-ins
  • No need to develop treatment plans or prescribe medication

Preferred Qualifications:

  • Active Nurse Practitioner license in New York State
  • MSN degree from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current BLS certification
  • At least 6 months of Nurse Practitioner experience preferred
