Expand your clinical expertise as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members across Carteret County. This per diem position allows you to set your own schedule, with shifts available any day of the week. Locations include Morehead City, Beaufort, Newport, and surrounding areasexpect flexibility and variety as you deliver essential assessments in patients homes.
Schedule:
- Flexible shifts offered Monday through Sunday
- Self-scheduling to fit your availability
- Choose your preferred commitment: minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete annual and post-discharge risk assessments in home settings, each visit averaging 45 minutes
- Review and evaluate patients medical history, conditions, and medication regimens
- Educate patients about their health conditions and medications
- Identify gaps in care and provide actionable recommendations for improvement
- No treatment planning or medication prescribing required
Required Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- At least 6 months of recent nurse practitioner experience preferred
In this role, you will be compensated $80$90 per evaluation, along with a mileage stipend and payment for administrative tasks.
