Job Description

Deliver exceptional patient-centered care in a dynamic per diem role focused on home health risk assessments. As an advanced practice clinician, you will visit Medicare Advantage plan members throughout Carteret County, including areas such as Morehead City, Beaufort, and Newport, conducting comprehensive health evaluations in their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule with customizable weekly or bi-weekly commitments

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical histories, conditions, and medications
  • Provide patient education on diagnoses and medication regimens
  • Identify care gaps and recommend appropriate actions or referrals
  • No treatment planning or prescribing responsibilities required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification as a Nurse Practitioner (AANP or ANCC)
  • Current CPR (BLS) certification
  • Minimum of 6 months of experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
