Job Description

Advance your career as a Nurse Practitioner specializing in home health risk assessments with a per diem opportunity. In this role, you will visit Medicare Advantage plan members within Howard County, Indianaincluding Kokomo, Greentown, and Russiaville. You'll be conducting comprehensive home-based assessments that support patient care continuity and improved health outcomes.

Schedule:

  • Flexible shifts available seven days a week, Monday through Sunday
  • Create your own schedule to fit your lifestyle
  • Meet a minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct annual health risk assessments in patient homes across Howard County
  • Review and document prior and current medical history, conditions, and medication use
  • Educate patients on health conditions and medications to promote understanding and compliance
  • Identify care gaps and provide recommendations for resolution to the care team
  • No responsibility for creating treatment plans or prescribing medications

Desired Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR (BLS) certification
  • Board Certification via AANP or ANCC
  • Six months or more of recent nurse practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
