Job Description

Deliver vital healthcare directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, practitioners perform in-depth annual or post-discharge risk evaluations for Medicare Advantage members residing throughout Tioga County, including Waverly, Owego, and Newark Valley. Each assessment typically requires about 45 minutes and includes a thorough review of each patients health background.

  • Flexible shifts available Monday through Sunday

  • Commit to hours on a weekly or bi-weekly basis

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members as scheduled
  • Evaluate patients medical histories, current health conditions, and medication regimens
  • Document health findings and educate members regarding their conditions and prescribed medications
  • Identify gaps in care or compliance and provide sound recommendations to enhance overall wellness
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Current Nurse Practitioner license in New York State
  • Active CPR certification (BLS)
  • Board certification via AANP or ANCC
  • Minimum six months of recent NP experience preferred
  • MSN degree from an accredited institution
