Job Description

Advance your clinical skills while making a difference for Medicare Advantage members by performing home health risk assessments throughout Tioga County, NY, including communities such as Waverly, Owego, and Newark Valley. As a Nurse Practitioner specializing in home health, you will have the autonomy to create your own schedule, with flexible shifts available between Monday and Sunday and the opportunity for per diem work. Enjoy a supportive structure that allows you to perform thorough, one-on-one health assessments in patients homes, ensuring high-quality care and meaningful impact.

Schedule:

  • Flexible scheduling between Monday and Sunday
  • Self-scheduled shifts
  • Option for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Perform annual and post-discharge risk assessments for Medicare Advantage plan members in their homes
  • Review prior and current medical history, medical conditions, and medication regimens
  • Offer education regarding medical conditions and medication adherence
  • Identify gaps in care and provide actionable recommendations to optimize patient outcomes
  • Visits averaged at approximately 45 minutes each
  • No prescribing or treatment planning responsibilities required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • Graduation from an accredited MSN program
  • Minimum of six months NP experience preferred

Compensation and Benefits:

  • Earn $80$90 per evaluation, plus a mileage stipend and paid administrative time for each assessment
