Job Description

Take your Nurse Practitioner expertise into the community with this meaningful per diem opportunity. In this role, youll complete home health risk assessments for Medicare Advantage plan members throughout Chemung County, including locations such as Elmira, Horseheads, Elmira Heights, and Wellsburg. Each risk assessment typically takes about 45 minutes and may include annual health evaluations or post-discharge visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate prior and current medical history, as well as all medication use
  • Provide clear education to members regarding medical conditions and medications
  • Identify gaps in care and make recommendations for further actions and follow-up
  • Focus exclusively on assessmentthere is no medication prescribing or treatment plan creation required

Desired Qualifications:

  • Active Nurse Practitioner license in New York
  • Current CPR certification (BLS)
  • Board Certified by either AANP or ANCC
  • MSN from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience preferred

Compensation for this opportunity offers $80-$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
