Job Description

A rewarding opportunity is available for an experienced Nurse Practitioner to provide home health risk assessments on a per diem basis. This role focuses on delivering in-home assessments for Medicare Advantage plan members in Elmira and throughout Chemung County, NYincluding surrounding areas such as Horseheads, Elmira Heights, and Wellsburg.

Schedule:

  • Flexible shifts available seven days a week
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments
  • Evaluate each members prior and current medical history, existing medical conditions, and medication usage
  • Educate patients regarding their conditions and medications
  • Identify gaps in care and provide actionable recommendations
  • Note: No treatment planning or prescription writing involved

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current BLS CPR certification
  • Board Certified via AANP or ANCC
  • MSN from an accredited nursing school
  • At least 6 months of current experience as a Nurse Practitioner is preferred

Candidates will be assigned to appointments within Chemung County as needed and may visit members homes in various local areas during individual shifts. For each completed evaluation, compensation ranges from $80 to $90, along with a mileage stipend and reimbursement for administrative time.
