Job Description

Make a meaningful impact by bringing essential care directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity empowers you to create your own schedule, with flexible shifts available between Monday and Sunday. Choose your preferred number of hours on a weekly or bi-weekly basis, allowing for greater control over your work-life balance.

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage health plan members
  • Review and evaluate prior and current medical history, medical conditions, and medication history
  • Educate patients about their medical conditions, medications, and preventive practices
  • Identify gaps in care and recommend next steps for closing those gaps
  • Visits may include annual health assessments or post-discharge follow-ups
  • No requirement to develop treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification via AANP or ANCC
  • Current CPR/BLS certification
  • MSN from an accredited school of nursing
  • At least 6 months of recent NP experience is preferred

You will be supporting patients across various communities, including Rocky Mount, Nashville, and Red Oak, within Nash County. Assignments and locations are flexible according to shift schedules.

This position offers competitive pay of $80$90 for each completed evaluation, plus a mileage stipend and compensation for administrative time.
