As a Nurse Practitioner specializing in home health, responsibilities are focused on conducting thorough risk assessments for Medicare Advantage plan members residing in Nash County, including communities like Rocky Mount, Nashville, and Red Oak. Each visit is scheduled at approximately 45 minutes, providing a chance to make meaningful impacts on patient well-being through annual health evaluations or post-discharge assessments.

Schedule:

  • Flexible shifts available seven days a week
  • Design your own work schedule
  • Minimum commitment options available (weekly or bi-weekly hours)

Primary Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and document prior and current medical histories, conditions, and medication use
  • Educate patients on diagnosed medical conditions and their medications
  • Identify and address gaps in care with recommended solutions
  • Prescribing medication and developing treatment plans are not required for this assignment

Required Qualifications:

  • Active Nurse Practitioner license in the state of North Carolina
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • Completed MSN degree from an accredited nursing school
  • Minimum of 6 months recent Nurse Practitioner experience is preferred
