A per diem opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage members throughout Ellensburg and broader Kittitas County. Youll have the flexibility to set your own schedule, with shifts available throughout the week. The primary focus of this position is conducting thorough annual health and post-discharge assessments to evaluate and support patient carewithout the responsibility of ongoing treatment management or prescribing.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own work calendar
• Minimal weekly or bi-weekly hourly commitments

Primary Responsibilities:

• Complete home health risk assessments, each lasting about 45 minutes, at patient residences.
• Review patients medical histories, current conditions, and medication regimens.
• Educate patients on their diagnoses, prescribed medications, and general wellness.
• Identify gaps in care and provide solutions or recommendations for follow-up.
• Locations may vary within Kittitas County, including Ellensburg, Connell, Kahlotus, Mesa, and nearby communities.
• No prescribing or treatment planning is required.

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• Graduation with an MSN from an accredited nursing program
• At least six months of recent Nurse Practitioner experience preferred
