Deliver high-impact care in a per diem Nurse Practitioner role performing home health risk assessments in Shelby County, TN. As a Nurse Practitioner specializing in Home Health, you will engage with Medicare Advantage plan members across Memphis and neighboring areas such as Bartlett and Collierville. Each assessment provides you a meaningful way to improve care while maintaining a balanced work schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingchoose your own availability
  • Set your minimum weekly or bi-weekly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Evaluate medical history, ongoing conditions, and medication usage
  • Educate patients on their medical conditions and prescribed medications
  • Detect care gaps and make recommendations to close them
  • No requirement for prescribing medications or developing treatment plans

Qualifications:

  • Active Nurse Practitioner License in Tennessee
  • Current BLS CPR certification
  • Board Certification from AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience (preferred)
