Deliver high-impact care in a per diem Nurse Practitioner role performing home health risk assessments in Shelby County, TN. This position offers a unique opportunity for clinicians seeking flexibility, autonomy, and a rewarding patient-focused environment. As a Nurse Practitioner specializing in Home Health, you will engage with Medicare Advantage plan members across Memphis and neighboring areas such as Bartlett and Collierville. Each assessment provides you a meaningful way to improve care while maintaining a balanced work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose your own availability
- Set your minimum weekly or bi-weekly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Evaluate medical history, ongoing conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Detect care gaps and make recommendations to close them
- No requirement for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current BLS CPR certification
- Board Certification from AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience (preferred)
