Job Description

A rewarding opportunity is available for a Nurse Practitioner with home health experience to deliver exceptional patient care and comprehensive risk assessments within a per diem role. This position focuses on conducting in-home health assessments for Medicare Advantage members throughout Rutherford County, including Forest City, Spindale, and Rutherfordton.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule according to your availability
  • Commitment can be set on a weekly or bi-weekly basis

Key Responsibilities:

  • Complete annual home health risk assessments during scheduled visits
  • Evaluate prior and current medical history, chronic conditions, and medication profiles
  • Offer education to patients on their health conditions and medications to empower informed decisions
  • Identify gaps in care and recommend solutions to improve patient outcomes
  • Visits do not include the development of treatment plans or the prescribing of medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR (BLS) certification
  • Board certification through AANP or ANCC
  • Minimum of 6 months current Nurse Practitioner experience preferred
  • MSN from an accredited nursing program

Compensation for this role consists of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
