A rewarding opportunity is available for a Nurse Practitioner with home health experience to deliver exceptional patient care and comprehensive risk assessments within a per diem role. This position focuses on conducting in-home health assessments for Medicare Advantage members throughout Rutherford County, including Forest City, Spindale, and Rutherfordton.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule according to your availability
- Commitment can be set on a weekly or bi-weekly basis
Key Responsibilities:
- Complete annual home health risk assessments during scheduled visits
- Evaluate prior and current medical history, chronic conditions, and medication profiles
- Offer education to patients on their health conditions and medications to empower informed decisions
- Identify gaps in care and recommend solutions to improve patient outcomes
- Visits do not include the development of treatment plans or the prescribing of medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months current Nurse Practitioner experience preferred
- MSN from an accredited nursing program
Compensation for this role consists of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
