As a skilled Nurse Practitioner, you will conduct comprehensive home health risk assessments for Medicare Advantage plan members across several communities throughout Wood County, including Parkersburg, Vienna, and Williamstown. This per diem position offers exceptional flexibility, allowing you to tailor your schedule to your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingcraft your own work calendar
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual risk assessments and post-discharge evaluations in client homes
- Review and document prior and current medical history, chronic conditions, and current medications
- Deliver patient-centered education regarding disease processes and prescribed medications
- Identify care gaps and advise on appropriate steps for improvement
- No requirement for developing treatment plans or prescribing medications
Desired Qualifications:
- Licensed Nurse Practitioner in West Virginia
- CPR certification (BLS) required
- Board Certification via AANP or ANCC
- MSN from an accredited nursing school
- Minimum of six months' recent nurse practitioner experience preferred
