Deliver comprehensive care as a Nurse Practitioner specializing in home health risk assessments for individuals enrolled in Medicare Advantage plans. This per diem opportunity is ideal for professionals who want flexibility while making a substantial impact in communities across Mercer County, including Charleston, South Charleston, and St. Albans.
Schedule:
- Flexible shifts available any day Monday through Sunday
- Control your own schedule
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior and current medical history, health conditions, and medication use
- Educate patients on existing medical conditions and medications to empower better health management
- Assess and identify potential gaps in care, providing actionable recommendations
- Note: This role does not require creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in West Virginia
- Current CPR certification (BLS required)
- Certified by either AANP or ANCC
- Minimum of 6 months experience as a licensed NP is preferred
- MSN degree from an accredited nursing school
