Job Description

Deliver comprehensive care as a Nurse Practitioner specializing in home health risk assessments for individuals enrolled in Medicare Advantage plans. This per diem opportunity is ideal for professionals who want flexibility while making a substantial impact in communities across Mercer County, including Charleston, South Charleston, and St. Albans.

Schedule:

  • Flexible shifts available any day Monday through Sunday
  • Control your own schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical history, health conditions, and medication use
  • Educate patients on existing medical conditions and medications to empower better health management
  • Assess and identify potential gaps in care, providing actionable recommendations
  • Note: This role does not require creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in West Virginia
  • Current CPR certification (BLS required)
  • Certified by either AANP or ANCC
  • Minimum of 6 months experience as a licensed NP is preferred
  • MSN degree from an accredited nursing school
