Advance patient care with a specialized focus on home health risk assessments. In this per diem role, you will utilize your Nurse Practitioner expertise to support Medicare Advantage members across Harford County, Maryland, ensuring comprehensive evaluations and improved health outcomes.

Schedule:

• Flexible shifts available between Monday and Sunday

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan members in locations across Harford County, including Aberdeen, Bel Air, and Edgewood
• Assess prior and current medical history, ongoing medical conditions, and medication regimens
• Offer patient education on health conditions and prescribed medications
• Identify gaps in care, reporting findings and providing actionable recommendations
• No responsibility for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner License in the state of Maryland
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC required
• MSN degree from an accredited nursing program
• Minimum of 6 months experience as a Nurse Practitioner is preferred
