Schedule:

• Flexible shifts available Monday through Sunday
• Self-determined schedule options
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members in their residences
• Review and evaluate past and current medical history, chronic conditions, and medication usage
• Educate patients on disease management and medication adherence
• Detect gaps in care and suggest actionable recommendations for improvement
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification from AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• At least 6 months of recent NP experience preferred

In this role, you will be compensated between $80 and $90 per evaluation, along with a mileage stipend and pay for administrative time, ensuring your efforts are valued both clinically and logistically.
