Deliver essential care and make a direct impact by conducting in-home health risk assessments as a Nurse Practitioner. This per diem opportunity focuses on serving Medicare Advantage members through personalized home visits within St. Joseph County, IN, including areas such as South Bend, Mishawaka, and Osceola. These evaluations help support optimal health outcomes and connect patients with necessary resources.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose a minimum commitment weekly or bi-weekly to fit your needs
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, chronic conditions, and current medications
- Deliver education and guidance regarding health conditions and prescription usage
- Detect gaps in care and offer recommendations for follow-up
- No treatment plan development or medication prescribing involved
Qualifications:
- Active Nurse Practitioner (NP) license in Indiana
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing school
- At least 6 months of recent NP experience is preferred
Nurse Practitioners in this role receive competitive pay between $80 and $90 per evaluation, in addition to a mileage stipend and compensation for administrative time. You will enjoy the autonomy that comes with per diem scheduling while playing a vital role in patient health.
