Job Description

Make a meaningful impact by performing comprehensive risk assessments for Medicare Advantage members as a Nurse Practitioner specializing in home health. In this flexible per diem opportunity, you'll play a crucial role in evaluating and educating patients across Beckham County, including Elk City, Sayre, and Erick.

Schedule:

  • Flexible shifts available seven days a week
  • Customize your work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete detailed annual home health risk assessments for Medicare Advantage plan members in residential settings
  • Review and document patient medical histories, current conditions, and medication regimens
  • Educate patients on diagnoses and prescribed medications, addressing questions and offering guidance
  • Identify care gaps and provide clear recommendations for follow-up or care improvement
  • Focus solely on assessmentsno treatment plan creation or prescribing required

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Board Certification (AANP or ANCC)
  • MSN degree from an accredited nursing school
  • Current BLS/CPR certification
  • At least six months of current nurse practitioner experience preferred
