Advance your nurse practitioner career by delivering essential care in a home health setting. This per diem role is ideal for clinicians seeking both flexibility and meaningful patient connections. The focus is on conducting risk assessments for Medicare Advantage members throughout Beckham County, which includes Elk City, Sayre, and Erick. Each visit lasts approximately 45 minutes and may involve annual health evaluations or post-discharge assessments.
Schedule:
- Flexible shifts available from Monday through Sunday
- Opportunity to set your own hours
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and document prior and current medical and medication histories
- Educate patients on chronic and acute medical conditions as well as their medications
- Identify care gaps and make recommendations to address them
- Visits are informational in nature; you will not be required to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of current Nurse Practitioner experience (preferred)
