Make a meaningful impact by performing comprehensive risk assessments for Medicare Advantage members as a Nurse Practitioner specializing in home health. In this flexible per diem opportunity, you'll play a crucial role in evaluating and educating patients across Beckham County, including Elk City, Sayre, and Erick.
Schedule:
- Flexible shifts available seven days a week
- Customize your work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete detailed annual home health risk assessments for Medicare Advantage plan members in residential settings
- Review and document patient medical histories, current conditions, and medication regimens
- Educate patients on diagnoses and prescribed medications, addressing questions and offering guidance
- Identify care gaps and provide clear recommendations for follow-up or care improvement
- Focus solely on assessmentsno treatment plan creation or prescribing required
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing school
- Current BLS/CPR certification
- At least six months of current nurse practitioner experience preferred
