Make a meaningful difference as a Nurse Practitioner delivering high-quality care through home health assessments for Medicare Advantage plan members in Union County, NJ. This per diem opportunity offers remarkable flexibility, allowing you to create your own schedule with shifts available Monday through Sunday. Serve individuals in areas such as Elizabeth, Plainfield, and Linden, conducting focused visits that typically last about 45 minutes.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Minimal hourly commitments weekly or bi-weekly options
Key Responsibilities:
- Complete comprehensive annual home health risk assessments in patient homes
- Review and document prior and current medical history, conditions, and medication usage
- Provide individualized education regarding medical conditions and prescribed medications
- Assess for care gaps and give actionable recommendations for follow-up or improvement
- No requirements for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Certified in Basic Life Support (CPR/BLS)
- Board-certified through AANP or ANCC
- Minimum 6 months of recent NP experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
