This per diem position offers Nurse Practitioners the flexibility to create their own schedules, delivering vital healthcare assessments and educational support to members in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose your own shifts
- Set your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review and document comprehensive medical, surgical, and medication histories
- Evaluate current and past medical conditions to identify care needs
- Provide education regarding ongoing medical conditions and medication management
- Identify potential gaps in care and recommend appropriate follow-up or resolutions
- Visits will not involve prescribing medications or developing detailed treatment plans
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current Board Certification through AANP or ANCC
- Six months of recent NP experience preferred
- MSN degree from an accredited program
- Valid BLS (CPR) certification
This per evaluation role compensates $80-$90 per completed assessment, and includes a mileage stipend as well as compensation for administrative time.
