Seeking an experienced Nurse Practitioner with a passion for home health risk assessments. This per diem opportunity empowers you to deliver high-quality, patient-centered care to Medicare Advantage plan members across Middlesex County, NJ, including Edison, Woodbridge, and Old Bridge. Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual home health risk assessments for members of a Medicare Advantage plan
- Review and evaluate medical histories, current conditions, and medication usage during each visit (each assessment typically 45 minutes)
- Educate patients on disease management, medication adherence, and preventive care
- Identify gaps in care and offer actionable recommendations to close them
- Visits may include initial annual assessments or post-discharge check-ins
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner license in New Jersey
- Board certification through AANP or ANCC required
- Valid CPR (BLS) certification
- MSN from an accredited nursing program
- At least six months of NP experience preferred
