Take the opportunity to make a significant impact in patients lives by conducting home health risk assessments for Medicare Advantage plan members within Hudson County, NJ. This per diem nurse practitioner position offers true flexibility, allowing you to create your own schedule and select shifts between Monday and Sunday, with a minimum commitment on a weekly or bi-weekly basis. Locations may include Jersey City, Bayonne, and Union City.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members in their homes
- Evaluate patients prior and current medical history, ongoing medical conditions, and medication usage
- Educate patients on their health conditions and proper medication management
- Identify care gaps and make recommendations to resolve them
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Current Nurse Practitioner license in New Jersey
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- Minimum 6 months of recent NP experience preferred
