Job Description

This per diem opportunity empowers you to create your own work schedule, conducting assessments for Medicare Advantage plan members throughout Union County, including Elizabeth, Plainfield, and Linden.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Set your own availability
  • Opportunities to commit hours on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform comprehensive annual and post-discharge home health risk assessments (average duration: 45 minutes per evaluation)
  • Review and document members medical histories, current health conditions, and medication regimens
  • Educate patients on their medical conditions and medications in a compassionate, clear manner
  • Identify care gaps and recommend appropriate next steps, collaborating with broader healthcare teams as needed
  • There is no requirement to create treatment plans or prescribe medications

Qualifications:

  • Unrestricted Nurse Practitioner license in New Jersey
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of six months' recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Nurse Practitioners in this per diem role are compensated $80-$90 per completed evaluation plus mileage reimbursement and compensation for administrative time.
