Enhance patient outcomes and deepen your impact as an experienced Nurse Practitioner specializing in home health. This per diem position focuses on conducting home health risk assessments for Medicare Advantage plan members across Morris County, including Parsippany-Troy Hills, Mount Olive, and Randolph. Each assessment averages 45 minutes, and may involve annual health evaluations or post-discharge visits within patients homes.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage members
- Review prior and current medical history, ongoing conditions, and medication usage
- Educate patients regarding their health conditions and medication regimens
- Detect gaps in care and recommend actions for resolution
- No responsibility for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- MSN degree from an accredited institution
- Board certification via AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months of recent experience as a Nurse Practitioner preferred
You will be compensated $80$90 per completed evaluation in addition to receiving a stipend for mileage and administrative time.
