Job Description

Advance your clinical expertise and make a meaningful impact by performing in-depth home health risk assessments as a Nurse Practitioner. In this per diem opportunity, you will support the well-being of Medicare Advantage plan members throughout Morris County, including Parsippany-Troy Hills, Mount Olive, and Randolph. Each visit averages 45 minutes and may be an annual health assessment or a post-discharge follow-up.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: set your own hours
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Perform comprehensive annual home health risk assessments in patient residences
  • Review and document medical history, ongoing health conditions, and medication use
  • Educate patients about their health status and prescribed medications
  • Identify care gaps and recommend resources or next steps to optimize patient outcomes
  • No requirements for treatment planning or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New Jersey
  • Current BLS CPR certification
  • Board certified by the AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience (preferred)
  • Master of Science in Nursing (MSN) from an accredited program
