Job Description
Deliver expert patient care on your terms as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers flexibility while allowing you to make a meaningful impact supporting Medicare Advantage plan members throughout various locations in Morris County, including Parsippany-Troy Hills, Mount Olive, and Randolph.
Schedule:
- Choose your own shifts between Monday and Sunday
- Flexible scheduling to accommodate your availability
- Set a minimum weekly or bi-weekly hourly commitment that aligns with your lifestyle
Key Responsibilities:
- Conduct comprehensive annual health risk assessments in members homes
- Review each patients current and prior medical history, ongoing conditions, and medications
- Educate members about their medical conditions and medication management
- Identify gaps in care and offer recommendations for care improvement
- Visits typically last approximately 45 minutes; responsibilities do not include diagnosing, prescribing medication, or creating treatment plans
Qualifications:
- Valid Nurse Practitioner license in New Jersey
- MSN from an accredited nursing school
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- At least six months of recent NP experience preferred
