Advance your clinical expertise and make a meaningful impact by performing in-depth home health risk assessments as a Nurse Practitioner. In this per diem opportunity, you will support the well-being of Medicare Advantage plan members throughout Morris County, including Parsippany-Troy Hills, Mount Olive, and Randolph. Each visit averages 45 minutes and may be an annual health assessment or a post-discharge follow-up.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: set your own hours
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform comprehensive annual home health risk assessments in patient residences
- Review and document medical history, ongoing health conditions, and medication use
- Educate patients about their health status and prescribed medications
- Identify care gaps and recommend resources or next steps to optimize patient outcomes
- No requirements for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current BLS CPR certification
- Board certified by the AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited program
