Job Description
Deliver exceptional patient care as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members across Middlesex County, NJ. This per diem opportunity lets you utilize your advanced practice skills in a supportive, flexible environmentideal for those seeking autonomy and variety in their workday.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Self-scheduling to accommodate your personal and professional life
- Commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct in-home annual health risk assessments, typically lasting about 45 minutes
- Evaluate patients' current and prior medical history, as well as medication regimens
- Educate patients on their medical conditions and prescribed medications during visits
- Identify care gaps and provide practical recommendations for improvement
- No requirement to create treatment plans or prescribe medications, allowing you to focus on assessment and education
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing school
- At least 6 months of recent NP experience preferred
