Advance your career as a Nurse Practitioner providing comprehensive home health risk assessments for Medicare Advantage members. In this per diem role, you'll play a critical part in promoting wellness and preventive care for members residing in various Middlesex County communities, including Edison, Woodbridge, and Old Bridge. Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual health risk assessments in patients homes, with each visit typically lasting 45 minutes
- Review prior and current medical history, assess medical conditions, and evaluate medication use
- Educate patients on their medical conditions and proper medication usage
- Identify care gaps and offer actionable recommendations
- No treatment planning or prescription authority requiredfocus strictly on assessment and education
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current BLS CPR certification
- Board certified through AANP or ANCC
- At least 6 months recent Nurse Practitioner experience preferred
- MSN from an accredited nursing program
Engage in meaningful patient care without the responsibility of ongoing case management or prescribing. For each completed evaluation, compensation ranges from $80$90, plus mileage reimbursement and compensation for administrative time.
