Job Description

Advance your Nurse Practitioner career by delivering essential home health risk assessments in a flexible, per diem position serving Medicare Advantage members. Your clinical expertise will have a direct impact as you visit members in their homes, with assignments throughout Hudson County, NJincluding Jersey City, Bayonne, and Union City.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to fit personal and professional needs
  • Enjoy minimum weekly or bi-weekly hourly commitment options

Key Responsibilities:

  • Travel to assigned homes to conduct comprehensive risk assessments for Medicare Advantage health plan members
  • Perform detailed evaluations of prior/current medical history, health conditions, and medication usage
  • Educate patients on medical issues, medication management, and preventive strategies
  • Detect any gaps in care and offer recommendations for follow-up and resolution
  • No requirement to prescribe medications or develop treatment plans

Qualifications:

  • Active NJ Nurse Practitioner License
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current Board Certification through AANP or ANCC
  • BLS/CPR certification
  • At least six months of recent NP experience preferred
