Advance your Nurse Practitioner career by delivering essential home health risk assessments in a flexible, per diem position serving Medicare Advantage members. Your clinical expertise will have a direct impact as you visit members in their homes, with assignments throughout Hudson County, NJincluding Jersey City, Bayonne, and Union City.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit personal and professional needs
- Enjoy minimum weekly or bi-weekly hourly commitment options
Key Responsibilities:
- Travel to assigned homes to conduct comprehensive risk assessments for Medicare Advantage health plan members
- Perform detailed evaluations of prior/current medical history, health conditions, and medication usage
- Educate patients on medical issues, medication management, and preventive strategies
- Detect any gaps in care and offer recommendations for follow-up and resolution
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active NJ Nurse Practitioner License
- Master of Science in Nursing (MSN) from an accredited institution
- Current Board Certification through AANP or ANCC
- BLS/CPR certification
- At least six months of recent NP experience preferred
