Advance your clinical skills in a dynamic per diem role focused on patient-centered care in a home health setting. As a Nurse Practitioner dedicated to home health risk assessments, you will support Medicare Advantage members throughout McHenry County, Illinois. Visits may include Crystal Lake, Algonquin, and Lake in the Hills, reflecting the rewarding and mobile nature of this position.

Schedule:

• Flexible shifts available Monday through Sunday
• The ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct in-home risk assessments for Medicare Advantage plan members
• Perform thorough reviews of patients' prior and current medical histories, conditions, and medication use
• Educate members regarding their conditions and medications during visits (approximately 45 minutes per assessment)
• Identify gaps in care and present recommendations to address them
• No prescribing or treatment plan developmentpurely assessment and education-focused

Qualifications:

• Active Nurse Practitioner license within Illinois
• Board Certification through AANP or ANCC
• Valid and current CPR (BLS) certification
• MSN from an accredited nursing program
• At least six months of current NP experience preferred

This opportunity provides compensation ranging from $80-$90 per evaluation, with additional mileage reimbursement and administrative time covered.
