Step into a meaningful role providing advanced care for Medicare Advantage members through home health risk assessments within Taos County. As a Nurse Practitioner specializing in home health, you will deliver comprehensive assessments that help improve outcomes for vulnerable populations across communities such as Taos, Questa, and Red River.
Schedule:
- Flexible shifts available seven days a week
- Self-scheduling allows you to build a calendar that fits your lifestyle
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Conduct thorough annual and post-discharge risk assessments in members homes, averaging 45 minutes per visit
- Evaluate medical histories, current health status, and medication regimens to paint a complete clinical picture
- Educate patients about their medical conditions, treatment options, and medication usage
- Identify gaps in care, providing actionable recommendations to close those gaps
- Focus exclusively on assessmentno requirement for prescribing medications or developing treatment plans
Qualifications:
- Active and unencumbered Nurse Practitioner License in New Mexico
- Board certification from AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- MSN degree from an accredited nursing program
- Preferably 6 months or more of recent Nurse Practitioner experience, ideally in home health or risk evaluation
