Take on a highly flexible and rewarding opportunity as a Nurse Practitioner providing home health risk assessments within Madison County, Illinois. This per diem position allows you to set your own schedule, with shifts available any day of the week to suit your availability. Youll play a vital role by conducting health assessments for Medicare Advantage plan members in the Edwardsville, Alton, and Granite City areas, helping to ensure continuity and quality of care in the community.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
In this role, you will:
- Perform annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate prior and current medical history, current conditions, and medication usage
- Educate patients on managing their medical conditions and medications
- Identify opportunities to address gaps in care and recommend appropriate next steps
- Visits average approximately 45 minutes for each risk assessment
- Participate in both annual assessments and post-discharge visits as needed
- Focus on evaluation and education onlyno prescribing or treatment plan development required
Desired Experience and Qualifications:
- Active Nurse Practitioner License in Illinois
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) degree from an accredited program
- 6 months of recent NP experience preferred
Compensation and Benefits:
- Earn $80$90 per evaluation, with an additional mileage stipend and compensation for administrative tasks
