Deliver high-impact clinical care with the flexibility and autonomy of a per diem schedule as a Nurse Practitioner focusing on home health assessments across Madison County, Illinois. In this rewarding role, you will provide home-based risk evaluations for Medicare Advantage members, contributing to preventive care and improved health outcomes without the need for ongoing treatment planning or medication management.
Schedule:
- Flexible shifts available any day of the week (Monday through Sunday)
- Customize your own schedule to balance personal and professional commitments
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive annual and post-discharge risk assessments in patients homes
- Review and evaluate prior and current medical history, conditions, and medication regimens
- Educate members on their medical conditions and prescribed medications
- Identify gaps in care and offer targeted clinical recommendations for improvement
- Visits last approximately 45 minutes each, providing time for meaningful patient interaction
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Illinois
- Valid Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current BLS-certified CPR credentials
- At least 6 months of experience as a Nurse Practitioner preferred
