Job Description
Deliver impactful care to patients in their own homes by conducting comprehensive health assessments for Medicare Advantage members throughout Oklahoma County. As a Nurse Practitioner in a per diem, home health setting, you will provide valuable insight into patients' health status and offer recommendations that help bridge gaps in careall while enjoying the autonomy of setting your own schedule.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Option to create a schedule tailored to your availability
- Minimum commitment required, either on a weekly or bi-weekly basis
Essential Responsibilities:
- Conduct thorough in-home annual health risk assessments, each averaging about 45 minutes
- Review and evaluate patients' medical histories, current conditions, and medication usage
- Deliver targeted education to members about their diagnoses and medications
- Identify care gaps and present actionable recommendations for improved outcomes
- Focus strictly on assessments and recommendations, with no prescribing or treatment plan requirements
Qualifications:
- Valid Nurse Practitioner license in Oklahoma
- Board Certified (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited institution
- Current BLS/CPR certification
- At least six months of Nurse Practitioner experience strongly preferred
Receive $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
